One more question: Do you know the form number or any other
information for the FSDO Wings application? The AC and the SPANS
site say to "send your application to the FSDO", but I can't seem to
locate a specific form to do so. At home I have some signed off
seminar attendance forms that include spaces for date / details / CFI
info for the flight portion. Is that the form? I'm confused, as they
don't look like typical official FAA documents.
I'm not sure what form is used where you are. The form we use certainly
isn't "formal". ;-) What you describe sounds like the form that is
needed. When in doubt, though, talk with someone at your local FSDO.
