Yes, English is _mandatory_ for pilots in the U.S.

But if i'm flying into Germany, for example, would I be speaking
to their ATC in german or english?


Your choice. Their ATC is supposed to be bilingual.

In a Spanish-speaking country, you'd have the choice of English or Spanish.
In any other country, you'd be required to speak English.
