What do people who fly into that airspace all the time do?

Flightaware.com, the website that provides free IFR flight tracking, also
provides common, ATC-accepted IFR routes between airports. It doesn't always
help since sometimes it only shows IFR routing for the kerosene burning
aircraft and other times doesn't have any recent routes, but it is a good
start for those parts of the country where "direct to" doesn't work. I always
start here.

Also, when flying from central NY to the west side of Manhattan, I have
discovered that ATC wants you on this route. I have received this from
Syracuse and Buffalo (the two times I received it from Buffalo I was able to
cancel and go VFR, since that route added 30 minutes to the trip):

HNK V167 WEARD V489 COATE

How you get to Hancock VOR is dependent on the departure airport - from
Syracuse it is V273.
