When you set up the flight, you can choose to start at any gate, or any
runway, or the active runway based on winds.

Do you have to
request engine start and go through start up sequences?


By default, engines are running at start. Shut down engines and save the
flight as default to start up "engines off" next time. Ctrl-E to auto-start
or most planes you can start by sequence.

If at the
gate do you get a push back?

How detailed are the airports -
buildings etc. or do you need the addons for more detail?


FS2004 adds taxiway signs which greatly improves orientation. Buildnig
locations are generally accurate but generic in appearance. Add-ons add
photorealistic buildings and scenery, plus details such as baggage trams,
service vehicles and real-world liveries.

Basically
I'm asking how realistic is it?
