I believe that the "official" checklist is FAA form 8000-38; you can
find an example of this form buried in FAA Order 8130.2. Here's a huge
link to a huge .pdf file containing 8130.2; the example of 8000-38
starts on page 197 of 285:

Over the last few years I've collected a few examples of how form
8000-38 was filled out so as to demonstrate "major portion"
eligibility.
