That is only true under some circumstances. There are exceptions, most
notably on a applicaton for a national security clearance.

Unless playing Euchre, I beleive in calling a spade a spade. If
it's a medical form it is stupid to ask non-medical questions beyond
the minimum needed to identify the appilcant (e.g. name, address,
history of name changes are not medical information per se).

If requiring other information, (like criminal records) can be
justified,
then that information should be requested separately and not lumped
in with requests for medical information nor shoudl anyone try to
go thorugh intelletualy dishonest mental gymnastics to try to justify
including such questions as part of a medical investigation.
