Default Aviation safety magazines

There was an aviation safety magazine that I used to read as a student
pilot that reported on accidents and fatalities from the previous month.
Investiation comments were included, and if there was any conclusion as
to the cause of the accident or fatality, it would be printed as well.

I'm sure there are a number of titles, but I can't remember any of them.

I'd like to get my hands on one again.

Can anyone help with some titles? Thanks,
