Haven't read the follow up posts yet but it happened at Farnborough
(spelling?) when they did a high speed pass with the newly uprated engines.
They pushed it harder than it had gone before.

Leading edge of wing delaminated and it was all over for the pilot (and
co-pilot?), there were no back seaters on board IIRC.

PS. scanned some replies, stories vary. Mine was sourced from a book I once
read, which doesn't mean it's correct either.
