Default Generic USB interface card

Anyone know of a generic USB interface card that will accept inputs from 4
pots, and 4 spst button switches, and report them to XPs 4 axis, and 4
button custom setup on the game port? Is there a better way to do it?

I would like to fabricate some life sized controls. I realize they are
already available, but most our so expensive I would perfer to make my own.
