Default Medical Form

Just got biannual checkup for my third class medical. A few months ago
I posted about getting my new airman's certificate number. The medical
form still has a field for the social security number (it's optional)
but no field for my new certificate number. I asked if this was the
most current form and I was told that it was. Anyway, I left the SS
number blank and did not include my certificate number. Seems strange
that they wouldn't want my certificate number. The Doctor agreed it's
a good idea to leave off the SS number if it's not required.

I did the same thing. As a general rule, I always write in NOYFB
in the box asking for my SSN. (Even on medical forms) Drives the
****ing insurance companies crazy.
