Default FAA Waiver / Security Statement

"MLenoch" wrote in message ...
Has anyone encountered these two forms? They require a statement of the
aircraft & route information and also the second page requires a statement of
the security handling of the aircraft. The forms do not have any identifying
numbers but are both dated "2-19-02".
Any value in completing these forms? Has anyone completed these for any
specific reason?

Only if you are seeking (and expecting) to get a waiver for one of the TFR's
or whatever. They are used for things like allowing medevac and traffic
reporters to operate in the affected areas.
